Community Care Services are social care services for adults who are vulnerable because of their age, illness and/or disability .
Our role is to help you get the support you need to live your life the way you want
We work in partnership with Berkshire West PCT, Berkshire Healthcare Trust and a range of local agencies and voluntary groups.
