Eligibility and Assessment

Reading Community Care Services are for:
- adults (18 years and over)
- who live in Reading
- and who are vulnerable because they are older and have become frail, have disabilities (physical disabilities, learning disabilities visual or hearing impairments or they are ill/have ongoing health problems (including HIV/AIDS and mental ill-health)
- and who need care and/or support services to help them live safely and independently.
