Community Care Services provide a range of care and support services to help adults with physical disabilities to help you live as independently as you can in the community. The support we offer depends on your individual situation - following an assessment of your care needs the support we offer could include things like:

  • Home Care - help at home with your personal care (such as getting out of bed, washing, dressing) or household tasks (such as shopping, paying bills)
  • Meals (through Meals on Wheels or lunch clubs)
  • Help in accessing community activities
  • Help in accessing education, training and work experience (if appropriate)
  • Occupational Therapy - specialist equipment and daily living aids to help you stay independent if you have a disability/mobility problems and help in accessing Disabled Facilities Grants to adapt your home to make it easier for you to get in/out and use the rooms/facilities in your home
  • Information, advice and practical help with issues such as getting the welfare benefits you are entitled to, moving into more suitable accommodation or arranging emergency alarm services
  • Emotional support
  • Helping you to access appropriate community health services such as a health visitor, district nurse or specialist therapies
