Check your eligibility for Community Care Services on-line

If you are older with mobility problems or you are and adult with physical disabilities you may benefit from care and support services to help you maintain your independence and stay in your own home for as long as possible. 

Use our on-line self-assessment form to see if you could be eligible for help from Community Care Services.  If you are eligible we will offer advice on the sorts of support and services that will suit your individual needs.

We can either arrange those service for you or, if you qualify for financial help from Community Care, we can give you the money we would generally have spent on your care so you can make your own arrangements - this is called Direct Payments.
