Nationally Local Authorities are responding to the government’s “Putting People First” agenda which sets out a vision and commitment to develop a more personalised approach to social care. Individuals who need support from Council’s should experience a reactive system that enables maximum choice and control with tailored support to meet their needs and goals.

In response to this Reading Borough Council’s Transforming Community Care Programme is focussed on radically rethinking the way we deliver care services to enable people to live their lives as they wish, confident that services are of high quality and are safe.

The picture below shows our vision for the transformed Adult Social Care and how it might work for you:

Enquiry/Referral StageServices Provided

1. Enquiries from the Public

All enquiries, whether received by phone, fax, email, letter or via website) are dealt with by Reading Adult Contact Team (ReACT)

All enquiries come through ReACT:

  • We provide good quality advice, information and signposting and for many cases no further intervention is needed
  • If you are eligible for short term or ongoing support from Community Care Services we will make a referral and start an assessment
  • We arrange simple services such as telecare, aids to daily living and Meals On Wheels
  • We make small adjustments to existing care arrangements

2. Referrals for short term support

Referrals for short term support are assessed for Intermediate Care or Reablement services

Intermediate Care and Reablement services offer up to six weeks of tailored and intense support and therapies designed to help you to regain your strength, mobility, confidence and independence after an illness, fall or hospital stay. The service may be provided in your own home, a residential unit or hospital setting. Many people require little or no ongoing support after this.

3. Referrals for ongoing support

Ongoing support includes:

  • Self Directed Support to help you stay living at home
  • Residential & Nursing Care

If you need ongoing support to help maintain your independence and live your life the way you want, you can decide what type of support you want and how it is provided (Self Directed Support). You can:

  • Ask the Council to arrange/manage this support
  • Have Direct Payments (we give you the money allocated to your care so you can arrange/manage your support
  • Have a combination of the two options

We will monitor and review your support with you. This is to ensure that the support you have chosen continues to meet your needs and goals.

If you have substantial needs that can't be supported at home we can help you find a residential or nursing home.

We are one year into our Transforming Community Care Programme to deliver the changes for Adult Social Care and have been focusing on the following projects:

  • Reading's Adult Contact Team (ReACT): One front door to manage all new enquiries efficiently ensuring the caller receives as much information and support in the first phone call as possible.
  • Intermediate Care & Reablement:  Reading aims to achieve a full Joint Intermediate Care and Reablement Service that helps older people and people with disabilities through a range of short term rehabilitative services, to maximise their potential for independent living.
  • Self Directed Support: The Self-Directed Support project focuses on encouraging people to have more choice & control of the support they need to live their life as they choose.
  • User Involvement & Consultation: With all the planned changes in Reading we recognise the importance of involving people who use services in the discussions about how services will look in the future. As a result a User & Carers forum has been established to directly feed into the Transforming Community Care Programme.
