Adult Social Care Services

Adult Social Care Services are for adults (18 years+) who need support because of their age, illness and/or disability to stay well and independent.

Our role is to help you get the support you need to live your life the way you want. Depending on your situation we can:

• Offer ADVICE AND INFORMATION and/or
• Refer you to our INTERMEDIATE CARE SERVICES and/or
• Refer you to our Adult Care Teams for SELF DIRECTED SUPPORT if you need ongoing care

We work with local NHS services and a range of support agencies and voluntary groups.
