When we place children for adoption we will automatically undertake an assessment of the child and adopter’s support needs prior to the placement being made. This will take into account the child’s previous history and the likely impact of this, as well as the child’s health, educational and contact needs. All prospective adopters will be provided with a Post-Adoption Support Plan which clearly explains the type and level of support they can expect to receive immediately following placement and in the three year period following the Adoption Order being made. This plan is reviewed annually by the Post Adoption Support Worker. After three years the responsibility for further assessment of support will fall to the local authority for the area in which the adopter(s) live.
