Team Reading

Adult care works to ensure that adults in Reading are well informed about the support services that they have access to. Assessment of people’s needs is also a very important part of our work, which in turn leads to the design and delivery of support and care packages. Partnership working is an integral part of what we do – working with organisations such as Health, Housing, local care providers and the voluntary sector to ensure that people have access to as much support as possible in their community.

Key services include:

  • Information and advice services are provided through initial contact into the Advice and Wellbeing Hub. A team of social workers and occupational therapists work with individuals and families to understand their needs, and identify the support they are entitled to.
  • Safeguarding provide guidance on safeguarding referrals that come into the department. Their aim is to make sure that individuals are protected from risk of harm or abuse and that practice through investigation and partnership working.
  • Provider services that deliver support and care directly to residents. These include Focus house (mental health supported living), Community Reablement Team, Shared Lives, Strathy Close (respite care), Maples (day care).

Our services support many different needs – often working with the most vulnerable people in the community and cover Supported Living, Day Services and Community Rehabilitation.

Public health and wellbeing

Reading’s Public Health and Wellbeing Service works to improve and protect the health and wellbeing of people in Reading, and to reduce health inequalities. The service works with partners to improve and protect Reading’s health and wellbeing, improving the health of the poorest fastest, helping to realise the vision of a healthier Reading.
