Personalised housing plan

A personalised housing plan is an agreement between you and the council. It explains what you and the council will do to help you take action and prevent homelessness. 

Your plan is based on your needs and situation. To follow your plan, you may have to complete tasks such as: 

• looking for suitable housing 
• providing documents or information we need 
• registering with housing providers 
• looking at private rented housing 
• getting support from other services 
• working with your homelessness prevention officer to help you find suitable housing 

Accessing your plan online 

From July 2026, the council have been able to issue personalised housing plans online. 

If the council have given you an online personalised housing plan, you can access it using My Reading Account.  

Simply sign in, and select the personalised housing plan link on the homepage. 

If you have a personalised housing plan, but it’s not online 

If you were given a personalised housing plan before July 2026, it won’t yet be available to view online. Please continue to follow it as discussed with your homelessness prevention officer. 

Your responsibilities 

You are responsible for completing tasks given to you in your personalised housing plan.  

If you are unable to complete a task or your situation changes, please contact your homelessness prevention officer as soon as possible. 

About online personalised housing plans 

An online personalised housing plan gives you access to your housing information 24 hours a day, 7 days a week. 

You can: 

• Access your plan at a time that suits you 
• View important target dates 
• See what tasks have been given to you and follow steps to complete them 
• Tell us when you’ve completed a task 
• Send messages to your homelessness prevention officer securely 
• Upload documents without needing to visit our offices 
• Keep track of progress on your case

If you are having difficulty finding your online personalised housing plan or need support, please contact your homelessness prevention officer.  

If your circumstances change or you are at immediate risk of homelessness, please contact us as soon as possible.
